Clinical and Electrocardiographic Studies on Chronic Cor Pulmonale

Abstract
Author studied 38 cases of chronic cor pulmonale from January 1971 to December 1976 at Korea University Hospital and the following results were obtained.
1. Male and female ratio was 1 1. 38. The age of most patients was 7th(47.4%) and less commonly in 6th and 8th decades(18.4% respectively)
2. Common causes of chronic cor pulmonale were chronic obstructive lung disease (75.8%) and pulmonary tuberculosis with fibrosis (21. 1%). 3. Cases of hematocrit levels over 45% were 66.6% in chronic obstructive lung disease and 33.3% in pulmonary tuberculosis with fibrosis.
4. Complications during admission were bronchopneumonia (18.4%), arrhythmia(18.4%), pleural effusion(13.2%) and CO2 narcosis(15.8%).
5. The prognosis of cor pulmonale was grave and 26.3% of cases were died during hospitalization.
6. The axis of P-wave and QRS complex were shifted to the right side and of T-wave. shifted -to the left side.
7. P-pulmonale was seen in 54.3% of cases.
8. The qR pattern was seen in 18.4% of cases in aVR, 10.5% in Vi and low voltage in standard leads in 26.4% and rS in Vs or Vs in 29.0%.
9. T-wave inversion in right precordiall lead seen in 34.2% and in all procordial lead in 21.1%, of cor
pulmonale.
10. By combined criteria of right ventricular hypertrophy, there were 63.2% of positive and 36.8% of false negative cases of right ventricular hypertrophy in cor pulmonale.
